Sampling Instructions

1. Print this page and completely fill in the spaces below.
2. Scrape approximately 1 teaspoon full of paint from the suspected surfaces - usually a window frame, door, and a wall per room. Scrape down to the substrate, getting all layers of paint.
3. Place each sampling point's scrapings in separate zip lock bags, remove air from the bags and seal well.
4. Write your name, telephone number, and sampling point description (e.g. "living room west window sill") on the bag with indelible ink.
5. Pack samples and this completed form in a secure box or padded envelope and send to:
